Yet theimpact of such structures is rarely explored. This article investigates the impact ofa career structure on the employment conditions of a Medical career grade in whichsome three-quarters of the doctors qualified overseas. The comparison of UK andoverseas qualified doctors in the Staff and Associate Specialist Group provides anunderexplored perspective on a professional career structure with a particularreference to reward, workload, autonomy and morale. The article draws on aquantitative study of 1,715 Staff and Associate Specialist doctors undertaken in2004. The findings demonstrate a complex comparative picture with overseasqualifieddoctors earning more, working for longer hours, having less autonomy and
a lower morale than their UK-qualified comparators. We conclude by pointing to thepotential for certain career structures to structurally and systematicallydisadvantage some groups.

This article investigates the impact of a professional career structure on the
employment conditions of a medical career grade in which some https://www.51lunwen.org/Medical/threequartersof the doctors qualified overseas. Career structures and their
associated boundaries supply and restrict career opportunities and provide thepotential to be sites of unfair discrimination and should be central to HRM theoryand practice. HRM has focused on the importance of ‘objective’ methods (such as job
evaluation) of determining differences between jobs for the purpose of reward todemonstrate equality proofing but has more rarely considered the impact of careerstructures in depth. The rationale for such structures often lies in pragmatic and
historical exigencies, although the competency approach seeks to overcome suchpractices.
This study of the impact of medical career structures is set in the National HealthService (NHS) whose employment relations are often characterised by employers asmade up of separate occupational ‘silos’ that reinforce professional tribalism (Bach,

(1997), we use the term ‘career structure’ to denotehistorically sustained patterns of occupational mobility where multiple individualactions maintain a dominant pattern, which becomes highly resilient to anychallenge.
