>a. Title Page – contains information related to the title of the thesis/dissertation, full name of researcher, the degree to which the research work is presented, the institution to which it is presented, and the month & year in which the proposal is to be presented.
b. Endorsement Sheet – states the recommendation of the thesis/dissertation adviser related to the oral defense of the research proposal, and contains the complete title of the thesis, full name of the researcher, degree to which the thesis/dissertation is applied for, statement of acceptance, adviser’s full name and signature, examination committee’s full name and signature, graduate dean’s full name and signature, as well as complete date of oral defense.
c. Table of Contents – lists the chapter headings into which the thesis/dissertation is divided and may include the following with their corresponding page citations: the preliminaries, chapter number, chapter titles, sub-heading of main headings, bibliography, appendices and curriculum vitae.
d. List of Tables – consists of the captions of tables included by the researcher in the report to provide substantial evidence in the presentation of the research results, and include the following: number of table, exact caption of the table title and page in the manuscript where the table is given.
e. List of Figures – consists of graphs, charts and other illustration materials used in the report, and gives the following information: number of figure, exact figure title, page in the manuscript where the figure is given.
B. Text of the Thesis/Dissertation
a. Chapter 1 – The Problem and Its Setting
1. Background of the Study – discusses the general context under which the research problem is to be studied, and indicates what the study is all about and the reasons for the conduct of the study.
2. Theoretical Framework – presents the set of theories that provide the backbone for the conduct of the study and drawn from the review of related literature and studies.
3. Conceptual Framework – shows the paradigm covering the conduct of the study that was conceived in the mind of the researcher, and based from the theoretical framework.
4. Statement of the Problem – states the specific interrogative statements, and as much as possible indicates the time, place and context of the research problems.
5. Statement of the Hypothesis – indicates the testable statement of what is expected to occur in the course of the research work, usually in the null form.
6. Significance of the Study – emphasizes the rationale for undertaking the study, describes the explicit benefits that will accrue from the study, and states the usage and intended recipients of the research findings.
7. Scope and Delimitation of the Study – scope defines where and when the study should be conducted and who will be the respondents, and establishes the study boundaries. On the other hand, delimitation refers to the phase or aspect of the investigation which may affect the results adversely but over which the researcher has no control.
8. Definition of Terms – presents the operational definition of variables or the functional definition, and explains the ways by which a given variable will be measured in the context of the study as well as the tools for the analysis and in the processing of the variables.
