加拿大课程作业:The effect of PRP of hospital doctors on hospital behavior: a case study from Shandong, China, Liu & Mills (2005)


This case study explores three different PRP systems sequentially used in Chinese public hospitals from 1980s to 1990s. There are two main aims of the case study. The first one is to investigate the different influences of these PRP systems on three operating aspects of public hospitals, which are revenue, cost recovery and productivity. And the second one is to identify the relationships between the probabilities of unnecessary health services and the three different PRP systems. The data of this case study is based on the information of six selected public hospitals of Shandong province in the period of 1978 to 1997.https://www.51lunwen.org/yyyxlw/

Since from 1980s, the Chinese government started to reduce the annual budget for public hospitals and allow them to earn profits by providing medical services. As a result, public hospitals have to pay more attentions on their administration and finances, because they need to earn more money to survive. Therefore the PRP systems were employed widely by them, in order to improve their revenue, cost recovery and productivity. 
In the case study, the three different PRP systems are called flat bonus, quantity-related bonus and revenue-related bonus. The flat bonus system is the earliest form of PRP system employed by Chinese public hospitals. By adopting this system, all the doctors of the hospital are paid equally, and the total amount of bonus is only related to the financial circumstances of the hospital. Thereby the flat bonus system could improve revenue, cost recovery and productivity of hospitals, but the effects might be not remarkable, because this system provides the weakest motivation for doctors to provide unnecessary health services to patients, in order to make more profits for the hospital. The quantity-related bonus system replaced the flat bonus system soon. By using this system, doctors will be paid bonus by achieving quantity goals, so they would have the passion of providing more unnecessary health services. Therefore this form of PRP system could be more effective than the flat bonus system for improving the financial performance of hospitals. Finally, nearly all the public hospital employed the revenue-related bonus system. This form of PRP system allows doctors to gain bonus according to the profits which they earned. Thereby the revenue-related bonus system could provide the strongest motivation for doctors to generate profits by providing unnecessary health services, meanwhile the financial performance of hospitals might be improved significantly.
The six selected hospitals have experienced all the three PRP systems. According to the data, the revenue increased dramatically, and the average annual increase rate was 16.3%. The cost recovery also increased. But there is a decrease in productivity, because the increase of patients is faster than the increase of capability of treating. Meanwhile, a large amount of expenditure for unnecessary health services can be found.
It can be concluded that by introducing PRP systems, the financial performance of public hospitals can be improved, but the probabilities of unnecessary health services will be also increased.
