>During Hospitalization:
All of the ADL’s of the patient was being altered. She had NGT in placed because she couldn’t swallow. She couldn’t dress and groom herself alone, and her mother was the one who did it for her. She defecated and urinated on her diaper, while she remained always lying on bed and unconscious.
Activity / Exercise:
>Before Hospitalization:
Playing with friends was the main exercise of the patient.
>During Hospitalization:
Since the patient can’t walk nor stand her usual activities was only repositioning, turning side to side, every two (2) hours to prevent skin breakdown.
Instrumental Activities of Daily Living:
Patient knows how to cook rice, wash dishes, simple house keeping like sweeping the floor. She is also the one who’ll prepare their table for their meal.
Social Data:
Social Support Person;
The patient is still dependent with her parents and has a good relationship with her family. They help one another in solving any problem they may encounter.
Socio-Economic Status:
Patient is not responsible and accountable to any socio-economic matter since she is still a child. Patient’s family belongs to a regular earning family. Her father is a fisher man that earns 150-200 pesos a day depending on the amount of catch and a member of Phil. Health, while her mother is a house wife that runs a mini sari-sari store with a daily income of 100-200 pesos.
Environmental Health Status:
Their house is not concrete but her mother claimed that the surrounding of their house was clean, so she was thinking that her daughter was really been infected from her school.
Ethnic affiliation
The patient’s family believes in “quack doctors” and “hilot”, and uses herbal medicines in treating their illnesses since they don’t have enough money to buy drugs/medicines.
Educational history
Patient is a grade 5 elementary student and has some difficulties in learning especially involving numeric.

Patient’s understanding was not assessed since the patient is in coma hence can’t response.
Client’s Ability to Follow Treatment:
Patient has no response during medication due to unconsciousness.
Health Beliefs:
The patient’s family believes in “hilot” and “quack doctors” but they believe most on the effectiveness of medical treatment.

GENERAL APPEARANCE:
Patient is unconscious and unresponsive. She appeared pale, weak, and couldn’t speak. She had an on going IVF of D5LR 60 mgtts/min infusing well; with Naso-Gastric Tube and O2 inhalation in nasal canula patent at 2-4 L/min.
SKIN:
• It’s generally uniform light brown in color
• Dry and warm to touch
• Poor skin turgor
• No edem
